PROLOGUE: The issue of drug spending has climbed back to prominence on the national agenda in a remarkably short time. Spectacular innovations, international pricing controversies, the question of Medicare coverage, direct-to-consumer advertising, and rapid drug spending increases at a time when other health cost growth was in check have all contributed to the galloping escalation of public policy interest.
