PROLOGUE: While environment and socioeconomic status influence health through a labyrinth of complex pathways, unhealthy behavior such as smoking, drinking, and illegal drug use has a relatively direct and unambiguous impact. Public policy regarding these types of behavior, however, is anything but simple. The policy debate is roiled by the emotional charge the issues have for those who have been touched by the ill effects of these activities and by the dicey complications that Americans’ views about individual liberty have on any attempt by government to regulate personal choice.
