Frequent Users of Health Services Initiative.
The "frequent users" targeted by this initiative include under- or uninsured people who are homeless; suffer from substance abuse; "have disabilities; have a chronic illness"; and/or have a mental illness with "serious health conditions," according to a June 2003 press release. The five-year, $10 million initiative aims to create "a cost-effective, comprehensive, and coordinated health care delivery system to improve care and reduce inappropriate use of high-cost emergency and [inpatient] services."
