PROLOGUE: As a new election cycle approaches, amid a plethora of anxieties ranging from fears of future terrorist attacks to the lethargic state of the economy, health care–related issues have once again vaulted to prominence in the public consciousness. Such renewed attention is hardly surprising, since private health insurance premiums rose 13.9 percent over the past year, topping a three-year run of double-digit increases. Although employers have not responded to such unwelcome news by moving in large numbers to drop employee health benefits, many workers have nonetheless been asked in recent months to shoulder an increasing portion of these premium . . .
