The ascendance of chronic disease in the U.S. health care "profile" has been widely reported in recent years. Statistics from the Centers for Disease Control and Prevention (CDC) tell us that upward of 70 percent of annual U.S. deaths are directly attributable to chronic diseases, including cardiovascular disease, cancer, and diabetes. Any lingering doubt about the level of policy horsepower that should be devoted to grappling with managing chronic disease should be dispelled by the staggering statistic that more than 75 percent of the annual U.S. medical care bill can be laid at the feet of these disease states.
