AS U.S. POLICYMAKERS contemplate major health system reforms, much of the focus is on expanding access for the uninsured and finding the dollars to pay for it. But the more than four in five Americans who already have health insurance face far different problems, many of which could be boiled down to one sentence: Its the delivery system, stupid. The many inadequacies in the way U.S. health care is organized and delivered require an overhaul.
