PROLOGUELooking Back, Looking AheadPROLOGUE: Initiative on health reform bounced back and forth continuously between Washington and the states in the 1990s against a backdrop of rising costs, growing numbers of uninsured, and constant tension over managed care waivers in Medicaid. Out of the controlled fury of these struggles came devolution and a new flexibility in state/federal relations. Among the first fruits of the new federalism was the State Childrens Health Insurance Program (SCHIP). While qualified by problematic interactions with welfare reform, SCHIPs successes appeared to suggest that a more sensitive equilibrium between state and federal roles could pave the way for further progress in covering the uninsured. Still, some difficult questions remain unanswered: how the burden of new subsidies might be distributed or how equity, accountability, and cost-effectiveness could be achieved? Each of the following three papers offers an overview of how the experiences of the recent past may condition the choices that lie ahead. Taken together, they present a picture of dangers and opportunities hanging in an excruciatingly suspenseful balance. "Catalytic federalism" is the expression coined by Larry Brown and Michael Sparer to describe the synergy that opened the way for SCHIP, by giving states the latitude and the money to design programs that pursued appealing goals but did not provoke crippling political antagonism. Alan Weil examines the unappreciated potential of incremental design decisions for influencing, and perhaps limiting, future strategies for further coverage expansions and applies this approach by analyzing a few aspects of competing proposals for tax credits and program expansions. Finally, Linda Bilheimer and David Colby review the history of coverage policy since 1980 with particular attention to the difficult trade-offs that have faced policymakers every step of the way, even under relatively favorable economic conditions.
