In the article by Tami Mark and Rosanna Coffey, "The Decline in Receipt of Substance Abuse Treatment by the Privately Insured, 1992–2001," Health Affairs 23, no. 6 (2004): 157 , the abstract contains an incorrect number. The last sentence should read as follows: "Substance abuse spending per covered life (in constant dollars) dropped from about $21.16 in 1992 to about $5.58 in 2001" (instead of "about $4.46 in 2002"). A corrected version of the article is available online at content.healthaffairs.org/cgi/content/abstract/hlthaff/23/6/157.

Also, in the article by Kenneth Chuang et al., "Independent Medical Review of Health Plan Coverage Denials: Early Trends," Health Affairs 23, no. 6 (2004): 163, Exhibit 3 contains several rows of incorrect numbers, in the section "Treatment category." A corrected Exhibit 3 is available online at content.healthaffairs.org/cgi/content/abstract/hlthaff/23/6/163. The corrected numbers also are reflected in a text change on page 165, seven lines above the subheading "Discussion." The text should read as follows: "When one looks at treatment modalities, IMR overturned more cases for gastric bypass than it upheld." Health Affairs regrets any confusion these errors may have caused.
