Pay-for-performance (P4P) articles seem to omit the crucial aspect of patient lifestyle and compliance. Why should a physician be judged on the control of hypertension, for example, when the great majority of the solution for this disease rests upon the shoulders of the patient? If no weight loss occurs, for example, the phsician should document this and not be subjected to any impact as a result. If the physician has documented that he or she has emphasized weight loss several times, it should add to their performance quotient, not delete from it.
