ORDERING PROCEDURE

There are several ways to subscribe to Health Affairs:
1. Order
online in our secure online ordering section and submit
electronically. -
2. Call toll-free 800-765-7514, in the United
States or 301-347-3900, outside the United States.
3. Fax your subscription request to 845-267-3478.
4. Mail
your subscription request and payment to Health Affairs,
P.O. Box 148, Congers, NY 10920 USA..

Once Health Affairs has received payment, you will begin
receiving the print journal. You also will receive an e-mail with
your Subscription Number and instructions for obtaining access to
the Web edition of the journal.
If you order online: you will have immediate access
to the online journal.
You will see a link on the Confirmation page, which will direct you
to the Home page for immediate access to online content. This
immediate access is a temporary bridge until you receive an e-mail (approximately 48 hours) with your Subscription number and instructions
for fully activating your online subscription.
If you order by phone, fax or mail: you will receive
a letter with your Subscription Number and instructions for activating
your online subscription. You may gain online access once you receive
your personal Subscription Number.
To gain online access to the journal
you will click on Activate
Online Subscription and will be asked to
provide the following information.

Your name, phone number, and e-mail address

Your subscription number (included with the e-mail)
